All football and cheer participants must first complete online registration by clicking “Available Programs” under the “Registration Info” tab 💻

Once you have registered online, please complete the required forms listed below.

📋 Required Documents

Prior to the first practice in August, all Marsh Creek athletes must submit hard copies of the following:

• 🩺 Physical dated after January 1, 2026, signed by a medical professional
  (Pop Warner Physical Form may be used, but is not required)
• 📄  Marsh Creek Emergency Treatment Authorization Form
• 📄  Marsh Creek Parental Requirements Form
• 📝 TWO COPIES of the final 2025–2026 report card
• 📣 Cheer only: Youth Participant Medical History Form 

⚠️ All items listed above are required before any athlete may participate in practices, for both football and cheerleading.

📍 Documents may be turned in at any pre-season registration event or during the first week of conditioning.
